Select the ICAO identifier as contained in the most current version of Doc 7910.
ICAO Operator Code*

Enter the operator’s three-letter ICAO identifier as contained in the most current version of Doc 8585. For general aviation aircraft, enter “IGA”. For military aircraft, enter “MIL”. If none, put XXX in this field and write the name of the operator/owner in the Operator Name field.
Operator Name*

Enter the Operator Name
State of Operator*
Select the ICAO identifier as contained in the most current version of Doc 7910.
Aircraft Type*
Select the ICAO Type designator as contained in the most current version of Doc 8643, e.g. for Airbus A320-211, enter A320; for Boeing B747-438 enter B744.
Aircraft Mark/Series

Enter the aircraft series or the manufacturer’s customer designation, e.g. for Airbus A320-211 enter 211; for Boeing B747-438, enter 400 or 438.
Serial No*

Enter the manufacturer’s serial number.
Registration*

Enter the registration mark of the aircraft with any dashes removed, e.g. for AA-XYZ write AAXYZ.
Mode S Address*

Enter the ICAO-allocated Mode S aircraft address code in hexadecimal format.
Date of withdrawal of RVSM Approval
MM slash DD slash YYYY

Enter the date of withdrawal of the RVSM approval in DD/MM/YYYY format e.g. for October 26, 1998, write 26/10/1998.
Reason for Withdrawal of RVSM Approval

Provide the reason for withdrawal of the RVSM approval
